Red Lake Family Health Team

 

The Red Lake Family Health Team offers high quality primary health care for residents in Red Lake and the surrounding area. An interprofessional group of health care practitioners, including family physicians, assisted by locums (short-term physicians that visit our community) provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs. The focus of the family health team is on disease management and prevention, rehabilitation, palliative care and health promotion.

Family Health Team General Information

A Family Health Team (FHT) is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you - the patient.

Family Health Teams are a key piece of the provincial government's plan to build a health care system that promotes good health, reduces wait times and improves access to care. In Family Health Teams, groups of health care professionals including family doctors, nurse practitioners, nurses, mental health counselors, dietitians and pharmacists work together to see patients and to keep them healthy.

Family Health Teams are an important part of changing both the health care system and the way people think about health care. FHT’s will focus more on preventing illness and promoting health. They will use proven tools to improve consistency, among members of the FHT, and to improve management of conditions such as diabetes and asthma. Family Health Teams will try to provide care that is more accessible and comprehensive. They will focus on making patients more involved in the management of their own health, commonly referred to as patient self-management.

About the Red Lake Family Health Team

The Red Lake Family Health Team is a non-profit corporation offering a community governed family health team. Patient enrollment is required to access these services. The family health team commits to offering quality primary health care in a collaborate setting that is aligned with our vision, mission, goals and values as follows:

The Red Lake Family Health Team will enable improved access to high quality, holistic primary health care in a multidisciplinary setting.

  • To recruit and retain health care providers who share a vision of a shared care model of health care delivery
  • To encourage clients/patients to be active participants in their own health care
  • Encourage the adoption of healthy lifestyles in a positive, supportive, non-judgmental environment
  • Make effective use of available community resources
