Client Population Served:
Adults (18+ yo) with:
- Type 1 Diabetes
- Type 2 Diabetes (diet controlled, pills, insulin, or any combination of treatment)
- Gestational Diabetes
- Type 1 Diabetes / Type 2 Diabetes & Pregnancy
- Impaired Glucose Tolerance (pre-diabetes)
- Impaired Fasting Glucose (pre-diabetes)
Children and Youth (under -18 yo) with:
- Type 1 Diabetes
- Type 2 Diabetes (on insulin)
- Type 2 Diabetes (no insulin)

How to Access the Program
- Self-referral
- Referral from Physician, or Nurse Practitioner
- Referral from Registered Nurse or Registered Dietitian
- Referral from Health Promotion Worker - Diabetes Prevention
- Referral from Homecare
- Referral from any other source

- Megan Doyle Gulliver RD, CDE Certified Diabetes Educator
The health care team also includes Family Physicians, a Nurse Practitioner and Registered Nurses who can provide foot care for patients with diabetes-related foot problems.
Hours of Operation
Monday: 11:00 AM - 7:00 PM (extended appointment hours until 7:00 pm)
Tuesday through Thursday: 8:00 AM - 5:00 PM
Friday: 8:00 AM - 12:00 noon
**Walk-in Clinic 8:00 AM - 11:30 AM - Tuesday Mornings with RN
**Closed during at noon for lunch & 3:00 PM - 4:00 PM on Mondays
Saturday & Sunday: Closed
